Skilled Nursing

We provide trained RN/LPN for your home health care needs. Services may include educating patients and family members about diseases, self-care techniques and prevention strategies. They do monitoring, documentations, assessment and evaluation of the patient’s physical and mental status. They also coordinate continually with your physician to plan and implement a complete home care treatment plan.

Speech Therapy

A licensed speech therapist assesses therapy needs and provides care accordingly. They coordinate with nurses and other rehabilitation staff to provide treatment towards the improvement of communication skills. Our therapist will continuously monitor and evaluate the patient’s progress.

Home Health Aide

A licensed home health aide provides personal care under the supervision of a nurse or therapist by following the Home Health Aide Care Plan. Home health aides coordinate care with the case manager.

Occupational Therapy

A licensed Occupational Therapist teaches the use of hands, fine motor skills, as well as cognitive skills to those challenged by stroke, arthritis, or other neurological problems. The physical therapist develops a home exercise program based on the patient's individual medical needs in accordance with the physician's orders. The occupational therapists helps improves the ability to perform tasks of everyday living.

Physical Therapy

A licensed physical therapist or a licensed therapy assistant assesses physical therapy needs and provide care accordingly. They coordinate with nurses and other rehabilitation staff to provide complete care by teaching home exercise, activity programs, monitor and evaluate patient’s progress.

Medical Social Services

Medical Social Worker (MSW) assists the patient with personal and financial situations. They provide short-term counseling services concerning the impact of physical, emotional, and psycho-social aspects of prescribed home health care plan.
